Job Description - Case Manager - Permanent Supported Housing (PSH)

Description

This is challenging, meaningful work that changes lives. As a Case Manager, you will support individuals experiencing housing instability as they work toward independence, stability, and improved well-being. While the work can be complex and fast-paced, it is deeply rewarding for those who are passionate about making a real difference in their community. You’ll be joining a team moving toward an emerging integrated care model in which housing, behavioral health, and physical health supports come together to provide truly whole-person care.

Responsibilities

  • Manage a caseload of participants while providing consistent, person-centered case management services
  • Spend time both in the office and in the community meeting with participants, supporting housing goals, and navigating barriers in real time
  • Conduct intakes, assessments, and develop individualized service plans tailored to participant needs
  • Provide regular in-person support—this may include home visits, office meetings, or accompanying participants to appointments
  • Support individuals in obtaining and maintaining stable housing, including working with landlords and housing providers
  • Assist participants with housing applications, subsidies, and navigating complex systems
  • Respond to urgent needs and crisis situations with professionalism, sound judgment, and compassion
  • Help participants navigate healthcare, behavioral health, justice, and social service systems as part of a whole-person approach
  • Build and maintain collaborative relationships with landlords, community partners, and service providers
  • Educate participants on tenant rights, responsibilities, and independent living skills
  • Balance direct service work with documentation, including case notes, service plans, and data tracking required for program compliance
  • Track outcomes and maintain accurate, timely, and confidential records
  • Participate in team meetings, supervision, and ongoing training

Requirements

Required: 

  • Eligible to obtain an agency Affiliated Counselor (AAC) credential within 30 days of hire
  • Valid driver’s license, reliable transportation, and required auto insurance
  • Ability to pass a background check and driving record review, consistent with applicable laws
  • Strong communication, organization, and problem-solving skills
  • Ability to work effectively with diverse populations in a fast-paced environment

Preferred: 

  • Bachelor’s degree in social services or related field
  • Experience working with individuals impacted by homelessness, trauma, mental health, substance use, or the justice system
  • Experience connecting clients to housing, medical, financial, or community resources

Compensation & Benefits

  • $28.41 per hour
  • 8 paid holidays plus up to 5 floating holidays
