Job Description - Case Manager - St. Anthony Family Shelter


Under the general direction of the Shelter Supervisor, provides general case management services including advocacy to the family unit. In addition to case management and advocacy, the Case Manager develops and delivers assigned programming to the adult/child Shelter population.

Schedule: Monday-Friday, 8am-5pm

Pay: starting at $22/hour

Accountabilities:

  • Complete the case management assessment, child and parenting assessments, and reassesses to determine progress and housing plan appropriateness. Initiate exit planning from first contact.
  • Develop a timely housing plan based on the findings of the assessment. Assure the family’s full participation with documentation and signatures.
  • Know and maintain professional contact with community resources. Make referrals, access resources with or on behalf of the family.
  • Monitor adherence to the Shelter’s Communal Living Agreement. Participate in extension and exit decisions. Participate in case staffing. Close cases when clients terminate from the program.
  • Meet agency requirements/standards for documentation.
  • Facilitate educational groups with shelter or outreach clients.
  • Collaborate with the Shelter Supervisor to determine programming gaps. Develop and deliver assigned programming to adults and children clients.
  • Demonstrate an understanding of, and support for, the mission of Catholic Charities and act as a representative of Catholic Charities in this role.
  • Serve as on-call supervisor on a rotating basis.
  • Perform other related duties as assigned.

Requirements:

  • Education
    •  Degree in social work, social sciences or other related fields preferred.
    • A combination of education and experience will be taken into consideration and may be substituted for degree.
  • Certification, Recertification, Registration, or License
    • Class C driver's license, motor vehicle, and insurance required.
  • Experience
    • Professional experience or training in domestic violence, economic disadvantages, homelessness, poverty, child development, mental health, disabilities, and working with children of all age levels.
    • Skills and Abilities
    • Experience using a computer to manage data
    • Familiarity with Microsoft Windows and Microsoft Office products.
    • Ability to maneuver through various computer programs including but not limited to searching for files, navigating, saving files, moving files, and printing documentable to use electronic database for maintaining client files
  • Exposures
    • Controlled inside work environment with noise/vibrations from general office equipment.
    • Frequent exposure to other environments such as schools, doctor offices, etc.
    • Minimal exposures to fumes/dust/odors.
  • Physical Demands
    • Work Type: Light, exerting up to 50 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
    • Climbing/Balancing/Freedom to Move: May be assigned to multilevel building with some floors accessible by stairs only. Minimal required moving about the office. Sit about 25 percent of the time.
    • Stooping/Bending: Regularly
    • Reaching: Occasional, overhead as well as horizontal and down.
    • Vision: Adequate to perform essential functions.
    • Color Vision: No particular color vision requirement.
    • Hearing: Frequently perceive nature of sounds by ear.
    • Speech: Frequently express ideas by means of spoken words.
    • Eye/Hand/Foot Coordination: Frequently operates equipment requiring moderate ability.
    • Manual Dexterity: Frequently operates equipment requiring moderate ability.
