Job Description - CCBHC Community Support Team (CST) Mental Health Professional

We are currently looking to fill a Mental Health Professional role at our Merrionette Park Center.


 SALARY:



  • The average starting salary for this position will fall in the range of $45,000 and $49,000 annually. Where candidates fall in this range will be based on skill and experience level. 


$2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment).


ESSENTIAL JOB FUNCTIONS:



  • Assist in conducting IM+CANS assessments and implementing treatment plans. Performs open access as assigned.

  • Provides case management, supports financial budgeting, assists with public benefits (Social Security, Medicaid), therapy/counseling, advocacy, health education and community support services.

  • Provides individual and group counseling, medication training and monitoring, skills training, and crisis intervention.

  • Responds to client and family needs outside of scheduled sessions, including during crises.  

  • Identifies and responds to changes in client needs, including decompensation or risk of hospitalization, and adjusts levels of care accordingly in consultation with supervisor. 

  • Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.   

  • Coordinates care with family members, internal providers (medical, dental, SA/DV), and external partners such as schools, DCFS, hospitals, and community organizations. 

  • Learns and utilizes agency and community resources.

  • Assesses community conditions affecting clients to identify resource gaps and problems.  

  • May serve on regional or agency-wide committees. 

  • Meets agency expectation for quantity of direct service and maintains quality and fidelity to evidence-based practices. 

  • Maintains timely and accurate client records and any necessary agency and CCBHC required reporting.   

  • Participates in agency and site based required meetings and trainings. 

  • Works effectively with diverse staff and service population. 

  • Other duties as assigned.









                KNOWLEDGE, SKILLS, and ABILITIES:



                • Requires mobility and willingness to work with a community-based caseload. 

                • Ability to maintain assigned community and home-based services. 

                • Ability to work effectively with diverse individuals and groups. 

                • Must work some evenings and weekends and possible on-call. 


                QUALIFICATIONS:



                • Bachelor’s degree in a human services field or a bachelor’s degree in any field with two years of documented clinical experience in a mental health setting, required. 

                • Must obtain and maintain IM+CANS certification, required. 

                •  Master's Degree in Social Work or Counseling from an accredited college or university. Must have applied to take LSW or LPC examination, preferred.  

                • Prior experience with Rule 132/140 or CST/ACT (Assertive Community Treatment) models strongly, preferred

                • Certified Alcohol and Drug Counselor (CADC) or experience working with clients experiencing substance use, preferred. 





                      ADDITIONAL REQUIREMENTS:



                      • Satisfactorily pass all relevant screening and criminal background checks.



                        PHYSICAL DEMANDS




                        • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
