Job Description - CCBHC Evaluation & Treatment Planning Specialist

We are currently looking to fill an Evaluation & Treatment Planning Specialist role at our Merrionette Park Office.


 SALARY: 



  • The average starting salary for this position will fall in the range of $64,000 and $72,000 annually. 

    • Final salary will be determined based on years of experience, licensure level, and relevant credentials 




$2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment).  


ESSENTIAL JOB FUNCTIONS:



  • Performs comprehensive, in-depth assessment and clinical formulation, development of accurate diagnosis and strengths-based treatment plans for CCBHC services.  

    • Utilize the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) to document clinical needs and strengths. 

    • Uses ASAM Criteria to evaluate clients across six specific dimensions, including withdrawal potential, biomedical conditions, and recovery environment. 

    • Facilitates the development of initial and updated treatment plans in collaboration with the client and their support system. 

    • Uses standardized criteria (e.g., ASAM for addiction) to determine the appropriate intensity of services. 









          • Performs open access as assigned. 

          • Collaborates with a multidisciplinary team, including Peer Support Specialists and medical staff to ensure seamless transitions and "warm handoffs". 

          • Provides crisis intervention, and case management services, adhering to CCBHC and MRO/Medicaid rehabilitation service models.  

          • Provides therapeutic interventions, advocacy, skill-building, and related services. 

          • Provides evidence-based treatment interventions per training and certifications. 

          • Learns and utilizes agency and community resources. 

          • Assesses community conditions that affect clients to identify resource gaps and problems. 

          • Works with multidisciplinary professionals including psychiatrists, nurses and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.   

          • May serve on regional or agency-wide committees. 

          • Meets agency expectation for quantity of direct service and maintains quality and fidelity to evidence-based practices. 

          • Maintains timely and accurate client records and any necessary agency and CCBHC required reporting.   

          • Participates in agency and site based required meetings and trainings. 

          • Works effectively with diverse staff and service population. 

          • Other duties as assigned. 


          KNOWLEDGE, SKILLS, and ABILITIES:  



          • Demonstrated superior skills with families or communities. 

          • Demonstrated competence in consultation, teaching, or writing. 

          • Requires mobility to work in the community with assigned caseload. 

          • Ability to work effectively with diverse individuals and groups. 


          QUALIFICATIONS:   



          • Master's Degree in Social Work, Counseling, Psychology, or a related behavioral science field. from an accredited college or university, required. 

          • Eligible or a Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), required.  

          • Must obtain and maintain IM+CANS certification, required. 

          • Must attend at least one training relative to the application of ASAM Criteria within the first six months of employment, required.

          • Final salary will be determined based on years of experience, licensure level, and relevant credentials, required.

          • 2+ years of post-graduate social work experience at an intermediate skill level, preferred. 

          • Certified Alcohol and Drug Counselor (CADC) or experience working with clients experiencing substance use, preferred. 

          • Experience with ASAM Criteria, preferred.

          • Licensed Clinical Professional Counselor (LCPC) or Licensed Clinical Social Worker (LCSW) in the State of Illinois, preferred.

          • IM+CANS certification, preferred.


          ADDITIONAL REQUIREMENTS:   



          • Satisfactorily pass all relevant screening and criminal background checks.


          TRANSPORTATION/TRAVEL REQUIREMENTS:   



          • ​​Driving for work required with transportation of clients preferred in a personally owned vehicle​. No travel between sites    


          PHYSICAL DEMANDS   




          • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
