The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. 

Jordan Health is now seeking a Chief of Urgent Care who, under the guidance of the Chief Medical Officer, is primarily responsible for providing health care for individuals in the Urgent Care Practice commensurate with training and in accordance with the general policies and standards of practice, as established by Jordan Health.

Requirements

The Chief of Urgent Care will ensure that:

· Health care services appropriate for an out-patient, primary care community health center with a multicultural patient population are provided respectfully and empathetically to Urgent Care patients.

· The highest quality of patient-centered care is delivered to patients.

· The physical, social and emotional health of patients and their families are promoted.

· Effective professional relationships are maintained. 

Education And Experience Required:

· Graduate from an accredited School of Medicine and post graduate training. Currently licensed to practice medicine in the State of NY. 

· Previous experience should be from an urgent care, hospital or ambulatory clinic setting. 

· Proficiency in MS Office Applications (Word, Access, Excel, Power Point, Outlook) and eClinicalWorks or other electronic health record. 

· Patience and tact to deal with the patient population and ability to work effectively in the primary care/urgent care environment. 

Licenses And Certifications: 

· Unrestricted license to practice in NYS

· DEA registration

· BLS through the American Heart Association 

· NYS Infection Control Certificate 

Special Skills, Knowledge Required:

· Knowledge of the principles and practice of modern medicine as related to public health organizations and community health programs as well as current social and economic problems pertaining to public health and their impact in primary health care. 

· Cognizance of the prevalent stressors and their impact in both the professional provider and the patient populations in the community health care primary care setting.

· Ability to work creatively with other health care professionals from a variety of disciplines to achieve maximum productivity.

Jordan Health is an independent FQHC, with Level III Patient-Centered Medical Home (PCMH) designation through the National Committee on Quality Assurance. Jordan Health receives funding from the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services. Jordan Health is a network of outpatient primary care offices with providers who follow a panel of patients.
