About RMBHS



Renewing Me Behavioral Health Solutions (RMBHS) is a CARF-accredited, Maryland-licensed behavioral health organization providing Outpatient Mental Health Services (OMHC), Psychiatric Rehabilitation Program (PRP), Substance Use Disorder (SUD) Treatment, and Intensive Outpatient Programs (IOP).


We are seeking a compassionate and dedicated Child and Family Resiliency Practitioner (CFRP) to provide psychiatric rehabilitation services to children, adolescents, and families within our PRP program.



Position Summary


The Child and Family Resiliency Practitioner (CFRP) provides psychiatric rehabilitation services to children, adolescents, and families enrolled in the Psychiatric Rehabilitation Program (PRP). Services are delivered in the office, home, school, and community settings and focus on improving emotional, behavioral, social, educational, and daily living skills.


The CFRP is responsible for completing new client intakes, conducting rehabilitation assessments, developing Individual Rehabilitation Plans (IRPs) with the child and family, providing direct rehabilitation services, coordinating care, and maintaining documentation in compliance with COMAR, Carelon, Medicaid, CARF, and agency requirements.



Essential Duties and Responsibilities



Intake and Assessment



  • Complete new client intakes and orientation to PRP services.

  • Conduct comprehensive rehabilitation assessments with youth and caregivers.

  • Identify strengths, barriers, needs, and functional impairments impacting daily living.

  • Collaborate with youth and families to establish meaningful rehabilitation goals.

  • Develop and update Individual Rehabilitation Plans (IRPs) within required timeframes.

  • Participate in annual reassessments and ongoing service planning.



Direct Rehabilitation Services



  • Provide psychiatric rehabilitation services in community, home, school, and office settings.

  • Assist youth in developing emotional regulation, coping, communication, socialization, and problem-solving skills.

  • Support development of independent living and community functioning skills.

  • Assist youth in achieving goals identified within the IRP.

  • Promote positive decision-making, self-esteem, and resiliency.



Family Engagement and Support



  • Engage caregivers and support systems in the rehabilitation process.

  • Provide education regarding behavioral health conditions and treatment recommendations.

  • Assist families with behavior management strategies and healthy communication techniques.

  • Encourage family participation in treatment planning and service delivery.



Care Coordination and Advocacy



  • Coordinate services with therapists, psychiatrists, schools, DSS, juvenile services, and other community providers.

  • Assist families in accessing educational, housing, recreational, vocational, medical, and social service resources.

  • Advocate for youth and families to promote successful community integration and treatment outcomes.



Documentation and Compliance



  • Complete service notes within agency-required timeframes.

  • Maintain accurate and timely documentation in ICANotes EMR.

  • Complete monthly progress summaries and required program documentation.

  • Participate in supervision, staff meetings, and multidisciplinary treatment team meetings.

  • Maintain compliance with COMAR, Medicaid, Carelon, HIPAA, CARF, and agency standards.



Schedule



  • Part-Time (20 Hours Weekly)

  • Hybrid Position

  • Community-Based and Office-Based Services

  • May require local travel to client homes, schools, hospitals, or community locations

  • Flexible Schedule with Afternoon and Evening Availability Preferred
