Client Care Manager

Scope of work:
Support the health, safety, and care of clients within the Host Home Program by managing medical needs, coordinating care across providers, and ensuring individuals receive consistent support aligned with their Dreams, Needs and Abilities (DNA) goals. 

Position Summary:
The Client Care Manager ensures that clients receive high-quality medical support and coordinated care within the Host Home Program. This role works closely with the host-families, healthcare providers, the day program, and the care team to ensure needs are met, documentation is accurate, and clients receive consistent support across all environments. 

The Client Care Manager also supports the organization's DNA framework by helping Host Home families and care teams implement goals related to client health, personal development, and daily living skills.
