Director of Wound Care - Nurse Practitioner

Job Summary

We are seeking an experienced and clinically strong Director
of Wound Care to lead our wound care service line across partnered
skilled nursing, assisted living, and long-term care facilities. This role is
responsible for both clinical oversight and operational leadership,
including advanced wound procedures, team mentorship, quality improvement, and
service line growth.

The Director of Wound Care will maintain clinical competency
in advanced wound care procedures, provide direct oversight of clinicians
performing these procedures, and own the performance metrics for the service
line. This position combines hands-on clinical work with strategic leadership,
requiring regular travel to partner facilities and collaboration with
cross-functional teams.

Key Responsibilities

• Lead
  the wound care service line, including clinical oversight, operational
  performance, and growth strategy.
  
• Maintain
  direct clinical competency in advanced wound procedures, including
  debridement and skin graft application.
  
• Oversee
  and mentor wound care clinicians performing sharp, mechanical, and
  enzymatic debridement, as well as skin substitute/graft procedures.
  
• Establish
  and maintain clinical protocols for wound assessment, staging, treatment
  planning, and escalation based on evidence-based guidelines.
  
• Ensure
  appropriate diagnostic workup is ordered, including labs, vascular
  studies, imaging, and cultures as clinically indicated.
  
• Support
  clinicians in identifying the underlying causes of wounds and factors that
  may impair healing.
  
• Monitor
  and improve clinical outcomes, documentation quality, procedure volume,
  and financial performance.
  
• Own
  and review the KPI dashboard for the wound care service line.
  
• Ensure
  accurate documentation, coding, and billing for wound care procedures.
  
• Collaborate
  with partner facilities to improve wound outcomes and reduce avoidable
  hospitalizations and emergency department visits.
  
• Build
  strong relationships with skilled nursing, assisted living, and long-term
  care partners.
  
• Support
  team development, retention, and certification compliance.
  
• Travel
  regularly to partner facilities to provide hands-on oversight, mentorship,
  and program support.
  

Required Qualifications

• Active,
  unrestricted Nurse Practitioner (NP) license in Kentucky,
  West Virginia, or Ohio; multi-state licensure strongly preferred.
  
• Current
  board certification in wound care (such as WCC, CWCN, CWS, or
  equivalent recognized certification).
  
• Minimum 5
  years of clinical wound care experience, including hands-on
  performance of debridement and skin graft procedures.
  
• Minimum 2–3
  years of clinical leadership, supervisory, or program management
  experience overseeing other clinicians.
  
• Experience
  providing wound care in skilled nursing, assisted living, or
  long-term care settings.
  
• Working
  knowledge of CMS documentation and billing requirements for
  debridement, skin substitute/graft application, and related procedures.
  
• Strong
  clinical judgment, leadership, communication, and organizational skills.
  
• Ability
  to travel regularly to partner facilities across multiple states.

Work Environment & Schedule

This role combines clinical fieldwork with program
leadership. The Director of Wound Care will spend time in the field supporting
partner facilities, performing or overseeing advanced wound procedures, and
mentoring clinical staff. Additional time will be spent on strategy, KPI
review, quality improvement, and coordination with leadership.
