About the Role
The Clinical Pharmacist is responsible for supporting the safe, effective, and appropriate use of specialty medications through drug utilization review, comprehensive clinical management, patient counseling, and clinical intervention.
This role provides direct clinical support to patients throughout their therapy journey, identifying and resolving medication-related concerns, educating patients and caregivers, and collaborating with healthcare providers to support positive outcomes.
The Clinical Pharmacist also provides oversight of patient care activities performed by pharmacy technicians and contributes to Birdi's quality improvement, patient safety, and clinical program development initiatives.
What You'll Do
Clinical Pharmacy & Medication Management
- Provide clinical pharmaceutical care in accordance with Birdi's Patient Care Program.
- Prevent, identify, and resolve medication-related problems.
- Make evidence-based, patient-centered medication therapy recommendations.
- Review prescriptions for clinical appropriateness, accuracy, and completeness.
- Perform Drug Utilization Review (DUR) for prescriptions prior to dispensing.
- Provide prospective, concurrent, and retrospective clinical interventions.
- Identify high-risk patients and provide appropriate documented clinical intervention.
- Maintain current knowledge of specialty medications, therapies, and administration systems.
Patient Counseling & Education
- Counsel patients and caregivers on medication and disease-state management.
- Educate patients on safe medication administration and proper medication use.
- Provide guidance regarding potential side effects and strategies for managing them.
- Conduct patient counseling at established points throughout therapy and when additional clinical support is needed.
- Provide drug information and education to patients, healthcare providers, and other members of the care team.
- Assist patients with coordination and referrals to external healthcare services when appropriate.
Prescription Verification & Patient Safety
- Verify prescriptions to ensure the correct medication and labeling are provided in accordance with company and regulatory requirements.
- Ensure quality and safety throughout the medication distribution process.
- Identify and appropriately report medication errors, adverse drug reactions, and medication recalls.
- Support safe medication practices and ensure clinical standards are consistently followed.
- Assume Pharmacist on Duty responsibilities when the Pharmacist-in-Charge is unavailable.
Clinical Programs & Quality Improvement
- Participate in the development and implementation of clinical programs.
- Contribute to disease management programs, clinical pathways, and drug utilization initiatives.
- Participate in quality improvement, performance improvement, and patient safety activities.
- Identify opportunities to improve clinical processes and patient outcomes.
- Support ongoing enhancement of Birdi's specialty pharmacy clinical services.
Education & Team Collaboration
- Serve as a clinical resource for pharmacy technicians and other internal team members.
- Educate internal staff regarding specialty medications, therapies, and clinical considerations.
- Mentor technicians and provide appropriate clinical oversight of patient care activities.
- Provide education or in-service presentations to external healthcare professionals when needed.
- Collaborate effectively with peers and other members of the healthcare team.
What You'll Bring
Required Qualifications
Education & Licensure
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD).
- Active Pharmacist license in good standing with the applicable State Board of Pharmacy.
Experience
- Minimum of 3 years of clinical patient care management experience, or PharmD with applicable clinical rotation experience.
- Pharmacy or healthcare industry experience, preferably within a mail-order or specialty pharmacy environment.
- Experience providing clinical medication management and patient education.
Knowledge, Skills & Abilities
- Strong clinical pharmacy and medication management knowledge.
- Excellent verbal and written communication skills.
- Ability to effectively communicate with patients, caregivers, healthcare providers, and internal team members.
- Strong clinical judgment, analytical thinking, and problem-solving skills.
- Ability to independently identify, research, and resolve clinical issues.
- Ability to mentor pharmacy technicians and collaborate effectively within a team.
- Strong organizational skills with the ability to manage multiple priorities and deadlines.
- Ability to exercise sound judgment and maintain patient confidentiality.
- Ability to work independently with minimal supervision.
- Ability to perform effectively in a fast-paced pharmacy environment.
- Proficiency with Microsoft Office and pharmacy-related software applications.
- Ability to work extended hours, weekends, and holidays as business needs require.
- Ability to participate in a rotating on-call schedule.
Preferred Qualifications
- Experience or knowledge of pharmacy compliance, regulatory, and accreditation requirements.
