The Health System Command Center Clinical Pharmacy Manager provides centralized clinical pharmacy services across the health system through a virtual command center model. This pharmacist supports inpatient, hospital-at-home, and ambulatory care environments by delivering remote medication order verification, medication reconciliation, clinical consultation, and tele-pharmacy services.
The role serves as a system-wide clinical resource, improving medication safety, supporting hospital capacity initiatives, and enhancing care transitions by ensuring accurate medication management across multiple sites of care.
This position plays a key role in supporting Hospital-at-Home programs, providing real time remote medication verification, patient counseling, clinical decision support and care coordination for care teams.
Remote pharmacists commonly review medication orders, monitor therapy, and intervene with providers to prevent medication-related problems across multiple facilities from a centralized hub.

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $159,000.00 - $190,565.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
