Description

The Health System Command Center Clinical Pharmacy Manager provides centralized clinical pharmacy services across the health system through a virtual command center model. This pharmacist supports inpatient, hospital-at-home, and ambulatory care environments by delivering remote medication order verification, medication reconciliation, clinical consultation, and tele-pharmacy services.


The role serves as a system-wide clinical resource, improving medication safety, supporting hospital capacity initiatives, and enhancing care transitions by ensuring accurate medication management across multiple sites of care.


This position plays a key role in supporting Hospital-at-Home programs, providing real time remote medication verification, patient counseling, clinical decision support and care coordination for care teams.
 

Remote pharmacists commonly review medication orders, monitor therapy, and intervene with providers to prevent medication-related problems across multiple facilities from a centralized hub.
 



Responsibilities

1. Remote Medication Order Verification

  • Perform centralized verification of medication orders across multiple hospitals and care sites.
  • Review medication orders for:
    • dosing accuracy
    • drug interactions
    • allergies
    • contraindications
    • formulary compliance
  • Collaborate with physicians, nurses, and pharmacy teams to resolve medication-related issues.
  • Ensure compliance with state regulations and health system medication safety policies.

2. Hospital-at-Home Clinical Pharmacy Support

  • Support Hospital at Home team in reconciling and ordering medications upon admission to Hospital at HomeReview and verify medication orders initiated by home-based providers.
  • Support remote monitoring of medication therapy.
  • Assist with medication management:
    • IV infusion therapies including but not limited to antibiotics, fluids
    • Medication monitoring: ex. anticoagulation, immunosuppressants, antibiotic dosing, etc.
    • chronic disease medication optimization
  • Provide telepharmacy consultation to Hospital at Home team 

Perform discharge medication reconciliation and support prior authorization processes as needed 

3. Transitions of Care Support

  • Perform admission medication reconciliation remotely using EHR tools.
  • Assist inpatient pharmacists with discharge medication reconciliation.
  • Identify discrepancies and medication-related problems.
  • Ensure continuity of therapy between:
    • inpatient care
    • hospital-at-home
    • outpatient care.

4. Patient Counseling & Tele-pharmacy

Provide virtual medication counseling for patients discharged from:

  • inpatient units
  • hospital-at-home
  • emergency departments.

Services include:

  • medication education
  • device teaching (inhalers, insulin, etc.)
  • adherence counseling
  • side effect monitoring
  • medication access support.

5. Post-Discharge Follow-Up

  • Conduct post-discharge follow-up calls (48–72 hours) for high-risk patients.
  • Assess:
    • medication adherence
    • prescription access
    • adverse effects
    • therapy questions.
  • Escalate issues to care teams when necessary

 

6. System Command Center Operations

  • Support centralized pharmacy operations including:
    • after-hours coverage
    • surge staffing support
    • clinical triage
    • medication safety alerts
  • Monitor health system dashboards for medication safety indicators.
  • Coordinate with local site pharmacists when clinical issues arise.

7. Quality & Performance Improvement

  • Participate in initiatives to improve:
    • medication reconciliation completion
    • discharge medication accuracy
    • readmission reduction
    • medication safety metrics.
  • Document clinical interventions in the pharmacy analytics platform.


Qualifications

Education

  • Doctor of Pharmacy (PharmD) or BS Pharmacy

Licensure

  • Active pharmacist license (state of New York)

Training

  • PGY1 residency preferred
  • PGY2 residency (ambulatory care, clinical pharmacy, or administration) preferred

Experience

  • 2–5 years hospital or health-system pharmacy experience
  • Experience with:
    • Epic / CPOE systems
    • remote order verification
    • telepharmacy
    • medication reconciliation

Preferred Qualifications

  • Experience supporting Hospital-at-Home programs
  • Experience in transitions of care programs
  • Board certification (BCPS / BCACP)
  • Familiarity with centralized pharmacy command center models

Compensation Statement

 

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $159,000.00 - $190,565.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
