What will you be doing in this role:
The Clinical Program Coordinator, RN, provides vital education and information on available options, the informed decision-making process, and realistic goal setting to empower the patient/caregiver to actively participate in their plan of care. The Clinical Program Coordinator, RN, establishes and coordinates a supportive care plan with goals and interventions based upon treatment and individual patient/caregiver needs, preferences, beliefs and values. The Clinical Program Coordinator, RN, reviews, evaluates and revises the plan of care for appropriateness on an ongoing and timely basis. Participates in the development of guidelines, protocols, pathways, and other performance improvement activities as it relates to patient care. Works in concert with the Patient Advisory Council to coordinate, implement, and evaluate educational programs/projects for patients and the community at large.
Hybrid Remote = must currently live in Los Angeles area or willing to relocate to LA area for onsite requirements
Experience Requirements:
Three (3) plus years of experience in an acute care setting (transplant, cancer, breast, pre-natal).
Solid understanding of Nursing Practice Act, The Joint Commission and other state and local regulatory requirements.
Preferred experience with program development and implementation of navigator programs.
Solid grasp of organizational program or project coordination/support experience; applying program/project management and development procedures.
Educational/License Requirements:
Bachelor’s of Science, Nursing (BSN) required.
Master’s degree preferred.
CA RN state License required upon hire.
ANCC Specialty Certification required (in area of specialty) within 1-year of employment.
