CNA PRN Hospice- Houston

Interim HealthCare, one of the nation’s leading providers of home care, is now seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. As a Hospice CNA, you will play a crucial role in delivering high-quality, personalized end-of-life care to patients in their homes, ensuring their comfort, dignity, and overall well-being during this sensitive phase of life.

Responsibilities:

Patient Care:

Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and monitoring vital signs.

Document any changes in the patient's condition and report concerns to the registered nurse or hospice team.

Medication Administration:

Assist in administering medications as per prescribed treatment plans and report any adverse reactions or side effects.

Maintain accurate records of medication administration.

Comfort Care:

Maintain a clean, safe, and comfortable environment, making necessary adjustments to ensure patient well-being.

Implement appropriate pain management techniques as outlined in the patient's care plan.

Documentation:

Maintain detailed patient records, documenting care provided, observations, and any changes in the patient's condition.

Ensure compliance with all documentation and reporting requirements as per hospice regulations.

Collaboration:

Work collaboratively with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to deliver holistic care to patients.

Participate actively in team meetings and contribute to care planning discussions.

Qualifications:

Licensed CNA in the State of Texas - required

1+ years' experience - preferred

Experience with Hospice or Home Health - preferred

Effective written and verbal communication

Ability to travel - must have reliable transportation

Clean background check and drug screen
