Prescott Nursing and Rehabilitation Community is located 20 miles from St. Paul.  We are a 65 bed facility that cares for short-term/transitional care/long-term Residents.  Our home is owned and managed by Atrium Living Centers.  Our Employees honor and respect each resident served while providing thoughtful and exceptional healthcare services in a safe, clean and friendly environment. • provide direct care to residents with activities of daily living.

1. 
   bathing, dressing, grooming, toileting, mouth care, nail care, positioning and transferring in and out of chair, bed.
2. report skin problems
3. provide active and passive range of motion as necessary.
4. perform vitals
5. promote resident independence by allowing the resident the opportunity and time to complete the activities of daily living.
6. answer call lights
7. assist residents with dining, record food and fluid intake
8. work with members of the resident care team to provide quality resident care.
9. direct or assist residents in transfer and ambulation techniques, with range of motion and with exercise activities.
10. use restorative techniques to promote resident independence and highest functioning level.
11. welcome new residents, and their families, new employees and guests
12. comply with Resident Bill of Rights

1. must have the ability to read and write in English, and follow oral and written directions.
2. must have a sincere caring and interest to work with seniors.
3. must be capable of regular attendance.
4. ability to relate positively to residents and their families, and with co-workers.
5. certified to work in the State of Wisconsin as a CNA
