Community Health Worker

Schedule: Monday - Friday, 8am-4pm; hours may vary

JOB DESCRIPTION:

EDUCATION
Minimum: High School Diploma or GED/HSE accredited by the State Board of Education
Preferred: Associates degree in social work or related field. Peer Support Professional Certification preferred.

EXPERIENCE
Minimum: 1 year of experience in social work or healthcare field
Preferred: 2+ years of experience in social work or healthcare field

LICENSE/CERTIFICATION
Peer Support Professional Certification required within 90 days of hire

JOB DUTIES
1. Coordinate logistical supports and circumvent barriers to treatment for patients
2. Care Management and Resource Utilization
3. Community Outreach and Education
