Job Description - Community Outreach Specialist/Liaison

COMMUNITY LIAISON JOB DESCRIPTION

JOB DESCRIPTION SUMMARY
Responsible for establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Works with the Clinical Supervisor to establish community education for clinical programs.
2. Establishes and maintains positive working relationships with current and potential referral and payer sources. 3. Builds and monitors community, customer, and payer and patient perceptions of the agency as a high quality
provider of services.
4. Maintains comprehensive working knowledge of the agency's markets including government agencies, major
payer groups, key referral sources, and competitor’s market positioning.
5. Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate
organization personnel.
6. Maintains comprehensive working knowledge of community resources and assists customers in accessing
community resources should services not be provided by the agency.

POSITION QUALIFICATIONS
1. Bachelor's degree in Marketing, Business Administration, or related field preferred, but not required.
2. At least three years' experience in health care marketing preferably in hospice care operations. 
3. Ability to market aggressively and deal tactfully with customers and the community.
4. Demonstrates good communications skills, negotiation skills, and public relations skills.
5. Demonstrates ability to take initiative to accomplish agreed upon goals.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT
1. Employees are asked to sign a statement indicating whether they can perform the essential functions of their

2. Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions
when kneeling, stooping, bending or reaching may be necessary.
3. The employee must be able to lift and/or move up to 25 lbs on a regular basis. There may be occasions in
which the employee must be able to lift or move up to 50 lbs.
4. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision,
depth perception and ability to adjust focus.
