Position Description:

We’re on the hunt for a Hospice Liaison who knows how to build genuine relationships with referral sources, communicate compassionately with families, and never backs down from a good parking lot conversation at an assisted living facility.

Position Requirements: 

• Explaining hospice with empathy, warmth, and not sounding like a textbook.


• Juggling facility visits, calls, family meetings, and coffee runs like a pro.


• Being a familiar, friendly face to our referral partners. 

Bonus points if you:

• Can find your way around a SNF blindfolded.


• Know that hospice isn’t giving up, it’s showing up differently.


• Have referral partners who already text you memes.
