Community Service Liaison - Cardiology -PRN

The Community Service Liaison supervises and coordinates all the education, promotion and cultivates the business of Cardiology services for the communities we serve. This position coordinates community activities of providers and team members. The Community Service Liaison assists in business development of referral sources and maintains positive relationship to ensure we are meeting customer expectations. This role provides the analysis of community outreach and referral patterns and potential market penetration tactics through designing education opportunities, physician to physician connections and direct to consumer activities for Baptist Cardiology awareness. The position assists staff members by identifying patient barriers assisting with treatment plans resulting in improved patient outcomes. The Community Service Liaison coordinates with BHC Discharge educators and social workers to ensure medication compliance through Pharmaceutical assistance programs. This role participates in developing and implementing agency administrative policy. Counsels clients individually as scheduled and in emergencies. The position provides training for team members regarding new or updated information as new policies, procedures and pharmaceutical guidelines. The Community Service Liaison represents agency in community or interagency activities. This role conducts or directs staff development and various public relations events.

• Assists staff members in analyzing barriers to patient care and in improving outcomes for the patients we serve.


• Participates in developing and implementing administrative policies.


• Works closely with Baptist discharge planner to coordinate follow up treatment and medications for self-pay patients.


• Works closely with clinical staff to identify therapeutic and affordable medications for patients.


• Requests Prior Authorizations from insurance as needed and updates staff on formulary changes and Prior Authorization requirements.


• Logs, creates inventories and monitors all pharmaceutical samples and disposes of expired samples in accordance with policy guidelines.


• Schedules on-site and off-site clinical staff education programs and physician community speaking engagements, and educational and community events.


• Coordinates participation in American Heart Association community events as well as community health fairs, BHVI Cardiovascular symposium, and Go Red events, Community programs and various public relations events.


• Maintains responsibility for grant writing for community education outreach.


• Participates on BHC Growth/ Outreach and Marketing committees to grow and market cardiovascular services.


• Assists in business development of referral sources and maintains positive relationship to ensure we are meeting customer expectations and assists with designing education opportunities, physician-to-physician connections and direct to consumer activities for Baptist Cardiology awareness.


• Identifies real/potential problems affecting the service and implements solutions with follow through and communication.

Minimum Education

 

• Bachelor's Degree Health Care Preferred

Minimum Work Experience

 

• 1 year experience and/or training in a health care setting Required

Required Skills, Knowledge and Abilities

 

• Knowledge of cardiac medical terminology and medications.


• Excellent written and verbal communication skills.


• Knowledge of Community Resources.


• Knowledge of health care market area (Primary Service area and Secondary Service Area).
