SUMMARY:

This role strengthens dementia capable communities across Southwest Washington by leading training, outreach, and program development efforts. This position partners with community organizations to expand access to culturally responsive dementia education and supports, with a particular focus on rural counties. The role builds collaborative relationships with healthcare providers, social service agencies, tribal partners, and other community gatekeepers while supporting programs such as Dementia Friends, Memory Cafés, and caregiver groups. This role manages program communications, contributes to statewide and regional initiatives, and ensures accurate data collection, reporting, and grant compliance. 

ESSENTIAL FUNCTIONS:

• Develop and deliver dementia specific training for AAADSW staff, contractors, and aging network partners, maintain a structured training plan with community partners, and identify training gaps to secure needed resources or subject matter expertise.
• Promote early diagnosis, advance care planning, and stigma reduction through targeted community education and outreach.
• Coordinate logistics for regional trainings, presentations, and outreach events, including scheduling, materials preparation, registration, communication with host sites, and follow up.
• Support individuals living with dementia to remain at home through the development of resources including early-stage programming, specialized supports to address safety and behaviors, and services to address complex needs.
• Build and sustain dementia friendly communities by developing partnerships with healthcare providers, social service agencies, first responders, tribal partners, faith-based organizations, and other community gatekeepers.
• Conduct targeted outreach to rural communities to expand access to dementia education, caregiver supports, and navigation services.
• Promote culturally attuned, linguistically appropriate, and accessible dementia services across diverse communities.
• Participate in statewide and regional dementia initiatives and collaborate with partners to optimize effectiveness and ongoing program improvement.
• Support development and expansion of dementia-specific programs such as Dementia Friends, Memory Cafés, caregiver groups, and early-stage engagement opportunities.
• Assist in building and coordinating a pool of trained facilitators and community leaders to sustain dementia friendly programming.
• Manage program communications, including outreach materials, partner updates, event announcements, and resource distribution.
• Participate in data collection, evaluation, and reporting for funders while tracking program activities, maintaining documentation, and supporting grant compliance through accurate monitoring of deliverables and timelines.
• Identify service gaps and develop new community resources while supporting continuous improvement of dementia related services through research, collaboration, community feedback, and quality improvement activities.
• Participate in agency meetings, trainings, and professional development activities, and assist with additional tasks or special projects as needed.
• Participates with other program staff to contribute to the development of the four-year area plan and procurement of services.
• Reviews and approves program expenditures and contractor invoices.
• Monitors contractor performance to ensure compliance with program standards, minimum service levels are met, contractors are spending to allocation and programs run efficiently and effectively. 
• Coordinates with Contracts Department to develop, review and evaluate Requests for Qualifications and Proposals. 
• Completes Contractor Risk Assessments and writes Comprehensive, Focused and Quarterly Monitoring reports.
• Reviews budgets and fund sources to allocate funding for program expenditures.
• Acts as the subject matter expert for assigned programs. 
• Implements and amends program policies, procedures and develops required reports. 
• Reviews Home and Community Living Administration Management Bulletins and develops draft communication to inform stakeholders.
• Collaborates with contracts to develop contract documents including the SOW, STC, and MOA’s.

WORKING CONDITIONS AND PHYSICAL EFFORT:

• When attending community presentations and fairs, the employee travels by automobile and is exposed to changing weather conditions.
• Work is normally performed in a typical interior/office work environment.
• While performing the duties of this position, the employee is regularly required to talk, hear, stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls, and reach with hands or arms. 
• The employee occasionally lifts or moves up to 20 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

MINIMUM QUALIFICATIONS:

• Bachelor’s Degree in Social Work, Human Services or related field and two years of experience providing direct human services. 
• Possession of a valid driver’s license and minimum state-required vehicle insurance and have use of reliable transportation.

KNOWLEDGE, SKILLS AND ABILITIES:

• Strong understanding of Alzheimer’s disease and related dementias, along with knowledge of Washington’s aging and dementia service systems, community supports, healthcare partners, and statewide initiatives.
• Skill in interpersonal communication, group facilitation, public speaking, culturally responsive engagement, and the ability to build effective partnerships across diverse communities, including rural and tribal partners.
• Strong organizational and project coordination abilities, including planning, prioritizing, developing procedures, managing multiple assignments, meeting deadlines, and working independently with sound judgment.
• Ability to lead or participate in meetings, trainings, work groups, and support groups, and to communicate effectively in person, by phone, and through written formats using clear, well-organized English.
• Proficiency with computers, Microsoft Office applications, virtual meeting platforms, and general office equipment, with the ability to maintain confidentiality and work at a desk using phone and computer for extended periods.
• Competence in data collection, reporting, basic program evaluation, and performing basic math functions such as calculating percentages.
• Ability to work collaboratively with diverse staff, contractors, and community members to establish and maintain effective working relationships.
• Ability to travel in your own vehicle throughout the five-county service area to meetings and community events, including locations that may not be ADA accessible.
