Crisis Case Manager

SUMMARY

The Case Manager provides community-based and administrative support to the Mobile Crisis Response team. The Case Manager assists clients with hospital engagement, discharge planning, crisis stabilization, provides linkage post hospitalization and to Outpatient Behavioral Health services. The Case Manager's work strengthens the continuity of care by creating a connection between hospitals and Pillars Community Health's follow up services while also providing additional operational support for crisis response.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Meets clients and families in hospitals, in the community, or at their home. Begins linkage documentation prior to discharge; obtains discharge summaries; and coordinate discharge planning, assists with post hospitalization referrals, discusses resources, crisis stabilization planning, and continued treatment options.


• Collaborates with hospital social workers and coordinates referrals for nursing services, substance use treatment, behavioral health, and other community resources. Serves as a liaison for PCH.


• Assist with continued post-crisis placement efforts in collaboration with other clinicians. In situations involving greater than 48 hours for placement, travels to the hospital to meet with the client, presents the assessment, gathers information about changes, communicates with the ER staff and physician. Discusses any changes, the reassessment, and safety plans with the Director.


• Receives and coordinates all incoming crisis referrals, including Authority and CARES referrals, and dispatches Mobile Crisis teams based on geography, urgency, staffing availability, and workload.


• Monitors team members' statuses and tracks active calls and team availability in real time; communicates response time updates to callers and escalates urgent situations as appropriate.


• Maintains communication between field teams, supervisors, EMS, law enforcement, hospitals, referral sources, and community providers.


• Opens client records, creates encounters, uploads referral documentation and safety plans, and completes required chart updates.


• Coordinates referrals with CARES and community partners and returns out-of-catchment referrals when appropriate.


• Attends hospital and other community stakeholder meetings as needed


• Gathers and inputs data into electronic health records according to department needs and compliance requirements.


• Maintains various logs and electronic client records in accordance with department and agency policies to help track and monitor key performance indicators and program outcomes


• Outreaches clients and client families and maintains all logs and tracking systems according to department needs


• Collaborates with other intake staff throughout the organization (Psychiatry, Outpatient Mental Health, etc) in processing client need for services.


• Participates in organizational meetings, in-services, monthly department meetings, and weekly staff meetings.


• As needed, maintain a small caseload to support case management needs of clients post-crisis services


• Other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE

Bachelor's degree in counseling, psychology, social work or a related field required. Equivalent combination of education and experience may be considered in lieu of degree.

1-2 years of direct client case management experience in social service, behavioral health, community health outreach, or related field required. Prior experience coordinating health care for clients/patients is preferred. Customer service experience required.

LANGUAGE SKILLS

Bilingual (English/Spanish) required.

Seeking candidate with a positive, caring, and compassionate attitude toward mental health and those living with mental illness. Must be able to problem solve and make quick decisions during intake calls. Must have excellent organizational and communication skills. Candidate must demonstrate initiative and be a self-starter. Candidate must be able to collaborate with other health care providers and work within different systems. Must be able to communicate effectively with emergency room (ER) staff and other mental health providers. Intermediate computer skills required.

OTHER

Must maintain a safe driving record and have a current valid Illinois Drivers License. Personal vehicle needed for travel to/from office and community-based interventions. Mileage for authorized work-related purposes is reimbursable.

PAY & BENEFIT INFORMATION

$20.00 - $25.00 per hour*
