The following is a questionnaire and employment/contractor inquiry, and authorization for background inquiry. Please complete it entirely so we can begin your application process. You will only be considered for a possible position when all forms are filled out completely.

Completion of this questionnaire and background check does not guarantee, imply, or contract you as a counselor or direct care provider. If you are chosen as a prospective provider, you will be notified by the administration to start the orientation process.

Position Overview
A Cognitive Connection is seeking compassionate and reliable Direct Care Providers to support children and adults with intellectual and developmental disabilities. This role focuses on assisting individuals with daily living skills, community engagement, and personal development in a respectful, supportive environment.

Key Responsibilities

• Provide direct support to individuals in home, community, and program settings


• Assist with daily living activities such as routines, social skills, and life skills development


• Encourage independence, confidence, and positive behavior


• Support participation in community outings and structured activities


• Follow individualized support plans and maintain accurate documentation


• Communicate effectively with supervisors, team members, and families


• Ensure a safe, respectful, and inclusive environment at all times

Qualifications

• High school diploma or equivalent


• Reliable transportation and valid driver’s license


• Ability to pass background checks and required screenings


• Strong communication skills and a compassionate, patient demeanor


• Experience working with individuals with disabilities is preferred, but not required (training provided
