Key Responsibilities

• Assist individuals with daily living tasks, including personal hygiene, meal preparation, and household chores.
• Support individuals in developing and achieving their personal goals outlined in their individualized support plans.
• Facilitate community engagement and social interaction, transporting clients to events and activities as needed.
• Maintain clear documentation of services provided and progress toward individual goals.
• Ensure a safe and supportive environment, identifying and addressing any potential risks.
• Collaborate effectively with families and other team members to provide holistic support.

• High school diploma or equivalent; experience in direct care or a related field is preferred.
• Exceptional interpersonal skills and a compassionate, patient demeanor.
• Strong ability to work both independently and collaboratively within a team.
• Valid driver's license and reliable transportation for outings with individuals.
• Availability to work flexible hours, including weekdays, weekends, and holidays.
• Current CPR and First Aid certifications are a plus, or a willingness to obtain them after hiring.
