Director, Case Management | Acute Rehab

We are seeking an experienced Director of Case Management to provide strategic and operational leadership for case management services supporting our acute rehabilitation programs at Inova Mount Vernon Hospital and Inova Center for Personalized Health in Fairfax. This leader will oversee case management operations, drive effective care coordination and transitions, and partner with interdisciplinary teams to optimize patient outcomes, access, and the overall patient experience.

The Director of Case Management leads and coordinates key strategic development and functional services to support department objectives and organizational goals. Maintains effective and responsive relationships with patients, attending medical staff and other departments. Manages department finances, including budgeting and control of salary, non-salary and capital investment expenses. Manages professional staff including day to day supervision, team member relations and staff development,

Job Responsibilities:
 

• Education - Bachelor's Degree

Preferred Requirements:

• CCM/ACMA certification 
• 2 years Progressive leadership experience in case management or care coordination within an acute rehabilitation care setting
