Spring Creek Rehab and Nursing is actively seeking a Director of Social Services for our Skilled Nursing Facility in Harrisburg, PA. The ideal candidate will have prior Long Term Care and managerial experience.

DUTIES:

• Directs, establishes & plans the overall policies/goals for the Social Services Department
• Responsible for assessments, care planning & helping residents adjust
• Familiar with a variety of the field's concepts, practices & procedures
• Relies on extensive experience & judgment to plan and accomplish goals
• A wide degree of creativity and latitude is expected
• Occasional Community Outreach
• Meet with residents to discuss the financial plan
• Gather documentation for Medicaid applications
• Collect Private funds due to the nursing home
• Assist with insurance authorizations
