Director of Waiver Services - Fort Smith

Bost, Inc. is seeking a full-time Director of Waiver Services in Fort Smith, Arkansas. Applicants must have BS/BA Degree or five years' experience in the Human Services/DD or related field and a minimum of three years supervisory experience. Applicants should also have a valid driver's license, reliable transportation, and proof of automobile insurance.

As the Director of Waiver Services, you will be responsible for the implementation, monitoring and modification of Waiver Services Person Centered Service Plans (PCSP) for individuals served, ensure the achievement of objectives specified in each PCSP, and ensure that licensure standards for Waiver Services are addressed and achieved. You will also be responsible for developing and maintaining the annual departmental budget, and track and maintain utilization for all plans of care. Director of Waiver Services also develops, maintains, and documents collected data regarding short-term and long-term departmental goals through the Business Improvement Plan (BIP), Dashboard, weekly reports, and the Outcomes Management Report (OMR), attends Person Centered Service Plans (PCSP) meetings of individuals served when needed, and ensures that individuals' Plan of Care (POC) are implemented. Other duties as assigned.
