Centra Behavioral Health Hospital is a brand-new 72 bed behavioral health facility that opened in 2026. We offer high-quality, compassionate care for a wide variety of patients in need. Our teams provide evidence-based, transformational care for a broad range of mental health concerns in a compassionate, comfortable environment.

How you'll contribute 

A Discharge Coordinator who excels in this role:

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge. 
• Creates and maintains up to date aftercare coordination community resources.
• Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.
• Provides resource information to patients and families as appropriate for future care needs.
• Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients’ treatment.
• Creates a detailed Discharge Summary and Contract for each patient as assigned.

 What we're looking for

Applicants should have a bachelor’s degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting. Additional requirements include:

• Prior experience with psychiatric and chemical dependency patients.   

• Unencumbered clinical license if eligible/needed per state regulations, preferred.  
• CPR certification and Crisis Prevention Training (CPI) within 30 days.

Hourly range: $22.20 - 28.68  per hour.
