Job Description - Discharge Planner (RN)- Bayview Med. Center

The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.


 


Location: Bayview Medical Center
Work Hours: Days

JOB REQUIREMENTS



  • Graduate of accredited nursing or allied health care professional program required. BS preferred

  • Licensure in clinical discipline (RN) in the State of Maryland & Washington DC

  • Active CPR 


 


WORK EXPERIENCE



  • Two (2) years home care or home care discharge planning experience preferred

  • Knowledge of Medicare Conditions of Participation

  • Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services.


 


Salary Range: Minimum 36.31/hour - Maximum 56.28/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
