Job Description - Disease Intervention Specialist (DIS)

Description

 

SUMMARY:

The DIS works as part of the interdisciplinary team to facilitate and coordinate clients’ continuum of care activities, ensuring optimum utilization of community resources, service delivery, and compliance with external agencies and referral sources requirements. The DIS facilitates disease intervention activities related to the prevention, screening, and treatment of sexually transmitted infections. The DIS works closely with community partners and state agencies to facilitate warm hand-offs and linkage to care. The DIS works in tandem with residential and outpatient case managers to provide wraparound services to clients in recovery. The DIS will report to the MAI Coordinator and work alongside the Case Manager Supervisors to maintain compliance with certifications and in-house procedures.

POSITIONS SUPERVISED:

DUTIES AND RESPONSIBILITIES:

· Provide disease intervention and linkage to care services for clients who are at high risk for or living with HIV, Hepatitis C, or related sexually transmitted infections.

· Provide case management services including HIV prevention education, care coordination services, referrals to appropriate resources including infectious disease testing, HIV/HCV treatment, PrEP/PEP, community supports, and other assistance programs.

· Assess the needs of the clients at program entrance and work with the client to establish goals for prevention and treatment of sexually transmitted infections.

· Continue to follow up with clients throughout service provision to ensure that basic needs are addressed, including required documentation, to allow the client to move forward and achieve goals.

· Liaison with community services and other case managers to assist the client with identified goals and needs through recovery.

· Educate the clients on harm reduction, promoting healthy resources, and risk reduction training to provide highest quality of person-centered care.

· Uphold client confidentiality in accordance with company policies and procedures and local, state, and federal laws and regulations

· Maintains case files and other required documentation according to agency policies while working alongside residential and outpatient case managers, other members of the medical team, and the Prevention & Recovery team.

· Ensure all GPRA documentation is completed in a timely manner

· Works with MAI Coordinator and grants department to ensure accurate data tracking, monitoring, and submissions to all appropriate agencies.

· May attend community-based events to promote health information and linkage to resources for prevention, screening, and treatment of sexually transmitted infections.

Requirements

MINIMUM QUALIFICATIONS:

Skills/Knowledge/Abilities:

1. Excellent written and verbal communication skills with the ability to effectively communicate with diverse populations.

2. Cultural and diversity understanding and awareness

3. Interpersonal skills that promote and encourage teamwork

4. Advanced knowledge of HIPAA and confidentiality

5. Advanced knowledge of behaviors and emotions related to addiction

6. Advanced knowledge of community and referral resources for client’s diverse needs

7. Skills in documenting effective case management notes

Education and Experience:

1. Bachelor’s degree (preferred)

2. At least 3 years’ experience providing case management to at-risk populations

3. Addiction treatment counseling: 1 year (preferred)

4. Valid NM driver’s license and clean driving record (required)

5. Case Management Certification (preferred)- Accredited Case Manager, Certified Case Manager, Certified Peer Support Worker, Community Health Worker, Certified Health Education Specialist, or related credentials.
