Early Intervention Service Coordinator

Position Overview:

We are seeking a dedicated and compassionate Early Intervention Service Coordinator to join our team. This full-time position plays a vital role in supporting infants and toddlers with developmental delays or disabilities and their families. The Service Coordinator serves as the primary point of contact for families, coordinating comprehensive early intervention services and ensuring seamless delivery of individualized support programs.

Primary Responsibilities

• Serve as the primary service coordinator for an assigned caseload of families with children eligible for early intervention services


• Conduct initial and ongoing assessments to identify family needs, priorities, and resources


• Coordinate and facilitate multidisciplinary team meetings, including Individualized Family Service Plan (IFSP) development and review sessions


• Ensure timely delivery of all required early intervention services as outlined in each child's IFSP


• Monitor service implementation and make adjustments as needed to meet changing family circumstances and child development
