Experienced Home PT

Join University Home Care Inc as an Experienced Home Physical Therapist (PT) providing high-quality, personalized care to patients in the Southeast Region of MI. We are seeking a compassionate and skilled PT to help improve the health and well-being of our clients through tailored therapeutic interventions and support.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.


• Provide skilled physical therapy services in patients’ homes to improve mobility, function, and quality of life.


• Document patient progress and report on outcomes to healthcare team and insurance providers.


• Educate patients and family members on exercises, equipment use, and safety precautions.


• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.

Requirements

• Licensed Physical Therapist in the state of Michigan.


• Proven experience in home health physical therapy preferred.


• Strong communication and interpersonal skills.


• Ability to work independently and manage a flexible schedule.


• Reliable transportation to travel to patient homes in the Southeast MI Region and surrounding areas.

Benefits
