Job Description - Family Medicine Physician


  

JOB DESCRIPTION: 

Exodus Healthcare Network is a two-location multispecialty clinic in the west part of the Salt Lake Valley. Providing quality care for a diverse population of patients. We are seeking a full-time dynamic board-certified family medicine physician to join our family medicine team in Magna and West Valley City, Utah where we provide family medicine services to those communities. Our hours of operation are the most convenient for our patients and provide an incredible opportunity to grow a practice.

Position Description

POSITION TITLE: Family Medicine Physician

PRIMARY FUNCTION: Provides quality, cost-effective medical care on behalf of Exodus Healthcare Network.

EDUCATION: 

  • GRADUATE EDUCATION: Graduate of an accredited      School of Medicine, US residency trained. Board Certified/Board Eligible.
  • CERTIFICATES/LICENSES: Utah Licensed MD/DO

EXPERIENCE: 

  • 2 years of Family Medicine experience preferred      but not required. Open to new resident graduates. 

LANGUAGE:

  • English
  • Prefer Spanish, but not      required.

INCENTIVE / BENEFITS PACKAGE

Exodus has the flexibility to tailor a benefit package which is unique to each and every Provider invited to join the Exodus Family.

Compensation:

  • Competitive salary and production bonus.

Retirement:

  • Exodus      Healthcare Network Profit Sharing Plan once they have completed twelve      (12) months of employment.
  • Employees can set aside (not      subject to Federal and State income taxes) the maximum allowable by whole      percentages of pay between 1% and 100% (4%, 5%, 8%, 10%, etc.). Exodus Healthcare Network will match      contributions up to five percent (5%). Match on the first three percent (3%) will be at one hundred      percent (100%) (dollar for dollar) and on the remaining two percent (2%)      at fifty percent (50%) (50 cents on the dollar) per dollar invested.      Employees are 100% vested in contributions and these contributions may not      be forfeited.

Medical / Dental Insurance:

· Employees who work 30 or more hours per week may apply for coverage with Exodus Healthcare Network group medical and or dental insurance. If elected, medical and/or dental coverage will start the month following 30 days of full time employment. 

Paid Time Off:

· Providers receive 15 days PTO Year 1 of service, 20 days PTO Years 2-4, and 25 days beginning Year 5. You may “carry-forward” up to five (5) days of PTO into the next calendar year. 

CME:

· Yearly CME allotment, $1,500.00

Dues and Membership:

· One national medical organization 

Licensing:

· Utah state medical license renewal

· DEA renewal fees

Malpractice:

· Malpractice insurance in the standard amount $1M/$3M during employment. 

  • No tail required.

Student Loan Repayment Program:

  • State      of Utah Primary Care and Rural Health eligible, based on funding cycle

Exodus Loan Repayment Program, periodic, based on annual budget and number of applicants

NATURE AND SCOPE:

Reporting Relationship:

  • Reports to the Medical Director of Exodus      Healthcare Network.
  • The incumbent works with all the departments of      the medical facility concerning all medical matters and keeps the Medical      Director informed of the operational status of the facility.
  • Consistent practice collaboration with      administrative team.
  • Number of others at same level reporting to the      same person: 8

Functions Performed Only by Incumbent:

Busy family practice and urgent care medical center, ability to care for patients of all ages, consistent with mission of Exodus Healthcare Network.

Production and Volume

  • Production and collected revenue driven practice.
  • Responsible for the development and growth of      practice to achieve financial goals based on support and overhead costs.

Provider Schedules and Location

· Clinic is open every day but Christmas day 7:00am to 10:00pm daily.

· Should expect to work one or more evenings per week.

· Should expect to work one of more weekend days per month.

· Clinic is open on all holidays except Christmas day.

· Possibly work one or more holidays in a year.

· Incumbent will work in both clinic locations in Magna and West Valley Utah.

· Schedules are flexible and may be twelve (12) or eight (8) hour shifts

Internal and External Contacts:

Interacts with all departments within Exodus Healthcare Network. Works closely with physicians, mid-level providers, practice administrators, and other staff at the facility


Requirements

  

· Responsible for diagnosing and treating all patients presented to the clinic while maintaining expected medical standards.

· Supervises mid-level providers while on-site at an Exodus Healthcare Network facility to which Employee is assigned. Advise management on medical deficiencies of medical auxiliary personnel.

· See patients in a timely and courteous manner.

· Refers patients with serious conditions necessitating inpatient care to appropriate specialty physician per criteria of federal, state, and managed care companies.

· Recognize the needs and desires of insurance contracts.

· Work with the staff in a positive, constructive manner.

· Advise Medical Director of administrative and medical deficiencies when noted.

· Contribute to the training and proficiencies of all clinical personnel.

· Abides by Exodus Healthcare Network dress codes (business casual) and Policies and Procedures not otherwise addressed in employment contract.

· Participates in quality assurance program that will assure that employees receive quality care in the center, and from other providers of healthcare service, using peer review, medical records audits, grievances, and other mechanisms as appropriate.

· Communication Skills: Effectively gives and receives information in individual and group situations. Customer Service: Makes the effort to listen to and understand the customer (Both external and internal); anticipates customer needs; gives priority to customer satisfaction.

· Safety Awareness: Awareness of conditions that affect employee's safety needs. Practices job using safety standards including Blood Borne Pathogens Program.

· Cooperation/Teamwork: Works as a cooperative member of a designated team.

· Initiative: Actively attempts to influence events to achieve goals; self-starting rather than passive acceptance. Taking action to achieve goals beyond what is necessarily called for; originating action.

· Policies and Procedures: Adheres to all policies and procedures including, but not limited to, attendance, dress code, continuing education requirements and confidentiality.

· Clinical Competence: Employee is responsible for the assessment, treatment or care of patients in the standard areas, as appropriate to the ages of the patients served.
