JOB OVERVIEW

• Job Title: Family Medicine

• Job Type: Permanent

• Location: Metro Atlanta/Suwanee, GA

• Service Setting: Outpatient

Coverage Period: ASAP - Ongoing

• Coverage Type: Clinical Only

• Shift Schedule: TBD; 4 to 5 days/week

PATIENT INFORMATION

• Patient Demographics: Adults 18+

• Patient Volume/Census: TBD

• Case Mix: Adult Primary Care, Preventive Care, and Chronic Disease Management

JOB REQUIREMENTS

• Active Georgia medical license (or ability to obtain)

• Board Certified in Family Medicine

- Board Eligible accepted only if currently in residency and boards must be passed before starting

• DEA

• Work Authorization: Must be authorized to work in the United States
