B3 Family Navigator

Position Summary

The B3 Family Navigator serves as the central liaison within a collaborative care coordination model designed to improve outcomes for children and families receiving services through LEO, Lynn Community Health Center, and Aspire Developmental Services. The Family Navigator provides comprehensive care coordination, family education, advocacy, and navigation services to an annual cohort of 20–25 high-risk families with young children.

Working across early education, healthcare, and developmental services, the Family Navigator partners with families to identify barriers to timely care, coordinate referrals and appointments, improve communication among providers, and ensure families understand and actively participate in their child's individualized care plan. This position supports a shared, cross-agency approach to reducing health disparities and improving access to services by addressing the social determinants of health.

This is a multi-year, grant-funded position through the Bright Beginnings Breakthrough (B3) Fund.

Primary Responsibilities

Family Assessment, Education & Advocacy

• Conduct comprehensive family assessments to identify strengths, priorities, developmental concerns, service needs, and barriers to accessing care and supports.


• Provide families with accessible, culturally responsive information and education to understand their child's development, health, and service needs; participate in decision-making; and advocate effectively for their child.


• Develop individualized care coordination plans in partnership with families and collaborating providers.


• Support families in advocating for their child and following through on recommended services and care plans.


• Identify and address barriers to accessing services (such as transportation, childcare, language access, scheduling, etc.) and other social determinants of health. Connect families with appropriate community-based resources.

Care Coordination & Navigation

• Serve as the primary liaison between cohort families, LEO, Aspire Developmental Services, healthcare providers, and case managers to coordinate services. Support families to navigate and follow through on individualized care plans and recommendations, including accessing and maintaining public benefits and health care coverage.


• Proactively monitor service connections and facilitate communication among providers to promote continuity of care, minimize duplication, and reduce the need for families to repeatedly provide the same information.

Partnerships, Documentation, & Continuous Improvement

• Facilitate communication among multidisciplinary care teams to ensure coordinated, family-centered service delivery.


• Participate in interdisciplinary case conferences and care planning meetings.


• Develop, conduct and assess family and staff surveys and assessment instruments.


• Maintain accurate, timely, and confidential program data documentation. Track referral completion, appointment attendance, care coordination activities, and family outcomes using shared data collection tools.


• Identify trends and barriers through ongoing data collection; contribute recommendations for continuous quality improvement.

Qualifications

• Bachelor's degree in Social Work, Public Health, Human Services, Healthcare Administration, Psychology, Early Childhood Education, or a related field required; Master's degree preferred.


• Minimum of three years experience in case management, family support services, healthcare navigation, early childhood services, or related human services.


• Knowledge of healthcare systems, developmental services, Early Intervention, early education, insurance programs, and community resources.


• Understanding of social determinants of health, health equity, trauma-informed care, and family-centered practice.


• Strong interpersonal, communication, organizational, and problem-solving skills.


• Demonstrated ability to build collaborative relationships across multiple organizations and disciplines.


• Experience using electronic databases, Microsoft Office Suite, Excel, and client management systems.


• Ability to analyze and accurately document data while maintaining confidentiality.


• Bilingual skills preferred
