Position Summary:
Under the direct clinical supervision of the Medical Director or Clinical Services Director, the Family Nurse Practitioner (FNP) delivers top-of-license, culturally responsive primary and preventative healthcare across the entire lifespan. Serving as a primary healthcare provider within a multidisciplinary team, the FNP conducts comprehensive health assessments, manages acute and chronic conditions, orders diagnostic testing, and prescribes appropriate pharmacologic and non-pharmacologic therapies.
Operating within the Delivery (The Gold Standard) framework, this role emphasizes high-touch patient coaching, preventative disease management, and active collaboration with specialized navigators and behavioral health professionals to improve health outcomes in the community.
