Job Description - FHP Coordinator


The Family Home Provider (FHP) Coordinator is responsible for the recruitment, onboarding, compliance review, and training process for Family Home Providers (FHPs). This position evaluates candidate and home qualifications, applies SCL, state, federal, and organizational requirements, and exercises discretion and independent judgment when determining whether FHPs meet standards to provide services. The FHP Coordinator partners with Operations, Human Resources, and Executive Leadership to support regulatory compliance, maintain required documentation, and recommend process improvements.

ESSENTIAL FUNCTIONS

  • Manage the Family Home Provider application process, review required documentation for completeness and accuracy, and      determine compliance with Supports for Community Living (SCL), state, federal, and organizational requirements.
  • Conduct pre-screening interviews with prospective FHP candidates, assess qualifications against program and regulatory      standards, and determine eligibility to advance in the onboarding process.
  • Lead panel interviews, evaluate FHP candidates, and make hiring recommendations for onboarding.
  • Conduct and review background screening results for FHPs and internal staff, including Nurse Aide Registry, Central Registry, Caregiver Misconduct, KARES, CAN checks, and SCL-required random screenings; interpret findings and determine candidate eligibility or need for leadership review.
  • Coordinate pre-hire and random drug screening and TB testing with Concentra for FHPs and internal staff. 
  • Responsible for submission of completed federal and state hiring documentation to the VersiCare Human Resources team for internal staff.
  • Conduct on-site home evaluations of prospective FHP homes, assess whether the home environment meets safety, program, and regulatory standards.
  • Assign      required training through the College of Direct Support, Human Development Institute (HDI), Thinkific, and other training      systems; monitor completion, maintain records, and identify ongoing training needs.
  • Manage the Thinkific Training Portal, develop or update training content as needed, monitor training completion, and recommend improvements to training processes based on provider needs and compliance requirements.
  • Oversee      completion of the FHP onboarding checklist, verify completion of required documentation and training, resolve or escalate deficiencies, and authorize the contractor's start date and independent functioning date when all requirements are satisfied.
  • Consult with and provide recommendations to the Executive Director regarding candidate qualifications, compliance      concerns, onboarding issues, and policy interpretations.
  • Maintain compliance tracking process for expiring FHP documentation, including driver’s licenses, auto insurance, CPR/First Aid, and TB documentation, and proactively coordinate renewal requests to prevent lapses in required records.
  • Coordinate distribution, collection, and filing of FHP performance evaluation records, route completed forms to the      appropriate parties and maintain accurate contractor files for compliance and administration purposes.
  • Partner with the Executive Director to develop, evaluate, and implement policies, procedures, and process improvements related to FHP recruitment, training, and onboarding.
  • Other duties as assigned.

Requirements

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS 

  • Primary activities are conducted within a well-lit, climate-controlled office.
  • Ability to travel to, enter, and navigate all assigned homes, offices, and community settings as required to perform essential job duties, with or without reasonable accommodation.
  • Ability to travel to homes, meetings, and community locations year-round, including during routine rain, snow, heat, and cold conditions.

EDUCATION AND/OR EXPERIENCE

  • Associate's or Bachelor's degree in Business Administration, Healthcare Administration, Human Services, or a related field preferred. Equivalent experience may be considered.
  • Two years of experience in supporting individuals with developmental disabilities is required.

KNOWLEDGE AND SKILL

  • Demonstrates proficiency and knowledge in supporting participants safely, adhering to their individualized person-centered care plans.
  • Capable of effective communication with residents and their families.
  • Displays strong communication and interpersonal skills, encompassing self-awareness, self-management, social awareness, and relationship management.
  • Utilizes discernment and critical thinking to resolve discrepancies and problems, exercising sound judgment, and involving appropriate parties in decision-making when applicable.
  • Possesses understanding of general business and office terminology, as well as basic knowledge of personnel policies, procedures, and employment laws.
  • Demonstrates intermediate proficiency in MS Office Suite products/programs, including the ability to compose routine correspondence and reports.
  • Maintains a valid state driver’s license.
  • Relates effectively to individuals across all levels and demonstrates teamwork by sharing responsibilities.
  • Handles unique situations professionally.
  • Exhibits strong planning, organizing, and prioritizing abilities, with a focus on efficiency and multitasking.

OTHER REQUIREMENTS

  • A clear state police criminal record check is required for any state in which the person has resided in the past year. If any prior convictions for offenses outlined in KRS Chapter 17.165 (1)-(3) or a prior felony conviction, employment work will not be offered. Additionally, annual background checks may be required.
