Responsibilities Include:

  Perform in-home initial assessments and reassessments for patients

  Develop, implement, and revise care plans in collaboration with interdisciplinary teams

  Provide training, support, and clinical guidance to Home Health Aides and Personal Care Assistants

  Educate patients and families on care, medications, and condition management

  Complete thorough and accurate documentation

  Participate in care planning and clinical team meetings

  
  


• 
  

  Ensure patient needs are met with quality, dignity, and compliance

  
  

Ideal Qualifications:

• 
  

  Active New York State Registered Nurse (RN) license

  
  


• 
  

  At least 1 year of nursing experience (home care experience preferred)

  
  


• 
  

  Strong communication and clinical assessment skills

  
  


• 
  

  Patient-focused mindset and ability to work independently

  Reliable transportation for travel between patient homes
