FNP or PA needed in Medina, NY

About the POSITION:
A well-established community health center in Western New York is seeking a dedicated Family Nurse Practitioner to join its team at a primary care site located in the heart of Medina. This full-time position offers a balanced schedule and the chance to work alongside a small, collaborative team of providers.

Schedule:

40 hour work week. Flexible schedule. Clinic is open:

• Monday, Tuesday, Wednesday, Friday: 8:00 AM – 5:00 PM

• Thursday: 8:00 AM – 8:00 PM

Responsibilities:

• Provide full-spectrum primary care services to a diverse patient population

• Collaborate closely with one other experienced FNP and an MD/DO in a team-based care model

• Conduct health histories and physical examinations

• Diagnose conditions and develop comprehensive treatment plans

• Prescribe medications in accordance with NYS guidelines

• Perform minor office procedures

• Provide counseling and patient education

• Refer patients to appropriate specialty care as needed
