Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Geriatric Case Manager in Naples, Florida.

POSITION OVERVIEW
• Community-based case management role serving older adults and their families
• Provide both short-term and long-term case management services
• Develop individualized care plans and coordinate community resources
• Conduct home visits and maintain ongoing client communication
• Help seniors remain independent and safely age in place
• Strong autonomy with support from a dedicated team

COMPENSATION
• Annual Salary: $55,000
• Fully Employer-Paid Health Insurance

SCHEDULE
• Monday-Friday Schedule
• 9:00 AM - 5:00 PM
• No Weekends
• No Call Requirements
• Community-Based Work Environment

ROLE
• Conduct client assessments and develop individualized care plans
• Coordinate healthcare, social services, and community resources
• Perform home visits and ongoing case management follow-up
• Advocate for older adults and their families
• Maintain documentation and care coordination records
• Support clients in maintaining independence and quality of life

• Bachelor's Degree in Social Work REQUIRED
• Master's Degree in Social Work Preferred
• Case Management Experience REQUIRED
• Experience Working with Older Adults Preferred
• Comfortable Conducting Home Visits REQUIRED
• Strong Communication and Organizational Skills REQUIRED
• Ability to Develop Individualized Care Plans REQUIRED
• Compassionate and Client-Focused Approach REQUIRED
