GOALS Program Manager

 Manages the Generating Outcomes and Liaisons for Students (GOALS) Program, which serves Pediatric, Adolescent, and Young Adult patients and their families. Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization.

Compensation: $24.00 - $30.00 per hour based on experience and qualifications 

• Support Pediatric, Adolescent, and Integrated Behavioral Health Departments with services for patients 4-21 years old with academic and neurodevelopmental concerns (ADHD, Autism, Learning Disabilities, and genetic syndromes as examples).
• Build and maintain relationships with local schools and district representatives to increase awareness of the program and learn about school support services available.
• Initiate GOALS Program enrollment through the clinic’s electronic health record.
• Educate patients and their families about the purpose of the GOALS program through patient outreach and in-clinic meetings.
• Contact schools, therapy agencies, and other healthcare centers to obtain additional records and information related to patient care as needed.
• Oversee patient and family communication and case coordination, as needed. Examples include explaining the GOALS program, facilitating appointments and school communication, providing resource handouts and web links, advocating for services, and working with PCC community health, social work, and other staff to optimize patient success.
• Manage the bimonthly psychiatry conference schedule and coordinate with department operations managers to ensure adequate PCC provider coverage.
• Oversee organization, tracking, and scoring GOALS assessment scales, including performing scoring.
• Lead the GOALS Community Advisory Council through selecting topics and speakers along with the GOALS Director by sending invitations, facilitating meetings, and taking meeting minutes.
• Plan monthly GOALS staffings and programmatic meetings and take meeting minutes.
• Assist with planning and orientation of any GOALS staff or health professional trainee onboarding.
• Provide GOALS Program presentations to clinic departments, teams, and outside organizations to enhance awareness and knowledge of the program’s offerings.
• Identify opportunities for improvement in program service, delivery, tracking, and workflow.
• Track GOALS patients through routine metric reports in conjunction with other PCC Operations staff. Report on metrics to GOALS and PCC staff as needed.
• Collaborate with Clinical Quality and Risk Management to ensure patients adhere to follow-up appointments in accordance with NCQA standards.
• Work with the GOALS Team, Population Health, Grants Manager, Finances, and other PCC Operations staff to identify opportunities for funding and reimbursement.
• Supervise and manage any assistants, interns, or volunteers with duties such as computer scoring, creating packets and notebooks, developing resource lists, and other activities as applicable.

Education: College degree in Social Work, Education, Public Health, Vocational Nursing, Community Health Work, or related field.

Experience: Minimum of two years of experience working in social, medical, education, human services, or community-based settings (may include internships).

Knowledge, Skills, and Abilities:

• Bilingual in English/Spanish preferred.
• Superb interpersonal skills to foster interactions with a diverse range of people of varying ranges of abilities and disabilities.
• Ability to think critically and solve problems effectively.
• Excellent communication skills, enabling the clear expression of thoughts both orally and in writing.
• Computer literacy, encompassing Microsoft Office in Word and Excel, information security protocols, scheduling, and email usage.
• Skills in developing and managing electronic reports, templates, and processes. Knowledge of electronic health records preferred.
• High-level organizational and time management skills with the ability to manage multiple priorities in a fast-paced environment.
• Competence in recruitment, supervision, and management of assistants, interns, and volunteers.

Certificate/License:

• Licensed or full certification in the field of study.
• Eligible for Community Health Work Certification training preferred.
