At CountryHouse Cumberland, a dedicated Memory Care community and part of Agemark Senior Living, we’re looking for an experienced and compassionate Health & Wellness Director – RN, CM/DN to lead our clinical team and help ensure exceptional, person-centered care for residents living with Alzheimer’s and other forms of dementia. What You’ll Do

• 
  

  Lead Resident Care: Oversee the health and wellness of residents while ensuring individualized, high-quality memory care.

  
  


• 
  

  Lead & Develop Your Team: Provide guidance, training, and support to nursing and care team members while promoting accountability and teamwork.

  
  


• 
  

  Serve as RN Case Manager/Delegating Nurse: Provide nursing oversight and delegation in accordance with Maryland assisted living requirements and standards.

  
  


• 
  

  Oversee Assessments & Care Plans: Complete and oversee resident assessments, service plans, and ongoing evaluations as needs change.

  
  


• 
  

  Manage Medications & Clinical Services: Oversee medication management, physician orders, treatments, and other clinical services.

  
  


• 
  

  Partner with Families & Providers: Communicate with residents, families, physicians, pharmacies, and other healthcare partners to coordinate care.

  
  


• 
  

  Maintain Compliance: Ensure clinical practices, documentation, and resident care meet company standards and applicable state regulations.

  
  


• 
  

  Support Community Leadership: Partner with the Executive Director and leadership team to create a safe, supportive, and engaging environment for residents and team members.

  
  

What You’ll Bring

• 
  

  Registered Nurse: Current, active RN license required.

  
  


• 
  

  Maryland CM/DN Qualification: Qualified to serve as a Case Manager/Delegating Nurse (CM/DN) in accordance with Maryland requirements.

  
  


• 
  

  Leadership Experience: Previous nursing leadership or management experience, preferably in memory care, assisted living, senior living, or long-term care.

  
  


• 
  

  Clinical Expertise: Strong knowledge of resident assessments, care planning, medication management, delegation, and clinical documentation.

  
  


• 
  

  Memory Care Mindset: A compassionate, person-centered approach to supporting residents living with Alzheimer’s and other forms of dementia.

  
  


• 
  

  Strong Leadership Skills: Ability to coach, develop, and hold team members accountable while building a positive culture.

  
  


• 
  

  Excellent Communication: Strong interpersonal, organizational, and problem-solving skills with a resident- and family-focused approach.
