Board: Board Certified (BC) in Hematology/Oncology
Licenses: Active Florida Medical License
Experience: Procedure log requirements met
Other Qualifications: Clean background and NPDB; Up-to-date CV and Lee Attestation Form required

DUTIES & RESPONSIBILITIES

Provide comprehensive outpatient hematology and oncology care
Collaborate with physician assistants, medical assistants, and registered nurses
Maintain accurate and timely medical records
Adhere to all credentialing and compliance requirements
Participate in procedure log documentation as required
