The HOA Housing Stabilization Case Manager is responsible for providing housing-focused support to older adults experiencing housing instability. This role involves connecting individuals with trauma-informed, culturally and age-appropriate services tailored to their unique needs. Duties include assessing housing and service needs, arranging, coordinating, and monitoring the delivery of individualized services to meet participants' needs. The Specialist may also provide crisis counseling, linkages to medical care, and assistance in overcoming barriers to housing stability. Primarily working daytime hours Monday through Friday, the position may require occasional evening or weekend adjustments with supervisory approval. The Housing Stabilization Case Manager reports directly to the Homeless Older Adults Manager.

• A passion for working with underserved populations and commitment to the Senior Services mission, core principles, and values.
• Skilled in active listening and interviewing techniques, such as Motivational Interviewing.
• Strong verbal and written communication skills with the ability to follow written and oral instructions and procedures.
• Knowledge of HIPAA regulations, ethical standards, and Person-Centered practices.
• Proficiency in Microsoft Office and web-based software programs; familiarity with HMIS preferred.
• Excellent organizational and planning skills to manage time effectively and meet deadlines.
• Adaptability and flexibility to work in a dynamic environment.
• Ability to work independently and as part of a team.
• Professional demeanor in interactions with participants, colleagues, supervisors, and external partners.
• Detail-oriented with the ability to multi-task and prioritize effectively.
• Strong problem-solving and conflict-resolution skills.
• Familiarity with community resources and elder-specific services is highly desirable.
EDUCATION AND EXPERIENCE
• Bachelor’s degree in social work, human services, or a related field. Equivalent combinations of education and experience may be considered.
• At least one year of experience in community-based services is required.
• Excellent customer service and telephonic communication skills
• Must possess a valid driver’s license, auto insurance, a good driving record, and pass an acceptable criminal background check
