Home Care Consultant

Reports to: Chief Operating Officer

Territory: Southeast Wisconsin 

Home Helpers is seeking a motivated and relationship-focused Home Care Consultant to help grow our agency through referral development, community outreach, lead management, and client conversion. This role works closely with prospective clients and their families from the initial inquiry through consultation and start of care while building strong relationships with healthcare and community referral partners.

Key Responsibilities

• Develop and maintain referral relationships with hospitals, rehabilitation and skilled nursing facilities, hospices, senior living communities, physicians, social workers, case managers, and other community partners.
• Generate new referrals through in-person visits, calls, emails, networking, educational outreach, and community events.
• Respond promptly to prospective client inquiries and manage leads through consultation and start of care.
• Conduct in-home and virtual consultations, assess client needs, explain services, and recommend appropriate care options.
• Maintain consistent follow-up with prospective clients and referral partners.
• Accurately maintain referral activity, lead status, next steps, appointments, and outcomes in the agency’s CRM.
• Work toward measurable growth goals, including consultations, new clients, service hours, revenue, and referral-source development.
• Collaborate with Scheduling, Recruiting, Client Care, Quality Assurance, HR, and leadership to ensure smooth client onboarding and service delivery.
• Travel throughout the agency’s Southeast Wisconsin service territory and provide coverage across markets as business needs require.

• 2+ years of experience in business development, sales, marketing, community outreach, healthcare, or a related field preferred.
• Home care, senior care, or healthcare industry experience is strongly preferred.
• Strong relationship-building, communication, consultative selling, organization, and problem-solving skills.
• Comfortable managing a sales/referral pipeline, using CRM systems, and working toward measurable performance goals.
• Ability to work independently while collaborating effectively with multiple departments.
• Bachelor’s degree in business, marketing, healthcare administration, communications, social services, or a related field preferred.
• Valid driver’s license, acceptable driving record, reliable transportation, and required automobile insurance.
• Willingness to travel throughout Southeast Wisconsin and occasionally attend early morning, evening, or weekend community events.
