Home Care/ Hospice Licensed Practical Nurse

University Home Care Inc is seeking a compassionate and skilled Licensed Practical Nurse to join our Home Care and Hospice team in Livonia, MI. This role involves providing exceptional nursing care to patients in their homes, supporting them and their families through a wide range of medical and emotional needs. The ideal candidate will be dedicated to delivering high-quality care while promoting dignity, comfort, and independence for every patient.

Responsibilities

• Provide skilled nursing care in patients' homes including medication administration, wound care, and monitoring vital signs.


• Collaborate with patients, families, and interdisciplinary team members to develop and implement individualized care plans.


• Educate patients and caregivers on care plans, medication management, and health maintenance.


• Document patient progress accurately and timely in compliance with company and regulatory standards.


• Respond promptly to patient needs and emergencies while maintaining professional and compassionate communication.


• Support hospice patients with comfort care, symptom management, and emotional support.

Requirements

• Valid Licensed Practical Nurse (LPN) license in the state of Michigan.


• Experience in home care or hospice nursing preferred.


• Excellent interpersonal and communication skills.


• Ability to work independently and as part of a multidisciplinary team.


• Reliable transportation to travel to patient homes within Livonia, MI and surrounding areas.


• Current CPR certification.
