The Role

We're looking for a compassionate Home Health Aide to provide hands-on personal care and support to patients in their own homes. This is an on-site role — you'll be working directly with patients and families every shift, following the care plan set by the supervising nurse.

What You'll Do

• Assist patients with activities of daily living, including bathing, dressing, and grooming.


• Prepare meals and assist with feeding as needed.


• Take and record vital signs, and report changes in patient condition to the supervising nurse.


• Perform light housekeeping related to the patient's care.


• Assist with mobility, transfers, and safe positioning.


• Provide companionship and emotional support to patients and families.


• Follow the individualized care plan established by the RN and care team.


• Complete required visit documentation accurately and on time.
  
  
  

What You Bring

• Current NJ Home Health Aide (HHA) or CHHA certification.


• CPR certification and up-to-date health screenings.


• Reliable transportation to reach patients' homes.


• Patience, compassion, and strong communication skills.
