As a PRN LVN with StellarCare, you will provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse, helping them maintain health, comfort, and independence.

Deliver nursing services in accordance with the patient’s plan of care, including medication administration, wound care, vital sign monitoring, and disease management support.
Reinforce education provided by the RN and assist patients and caregivers in managing chronic conditions and daily healthcare needs.
Observe, document, and promptly report changes in patient condition to the supervising RN and physician as appropriate.
Complete timely and accurate visit notes and required clinical documentation through our EMR system.
Work closely with RNs, physicians, therapists, and the StellarCare clinical team to ensure coordinated, patient-centered care.

Graduate of an accredited vocational nursing program with a current, unrestricted Licensed Vocational Nurse (LVN) license in the state of practice.
Minimum of 1 year of nursing experience required; Home Health experience preferred.
Valid driver's license, current auto insurance, and dependable transportation.
Comfort with electronic documentation, mobile communication, and EMR systems.
Strong patient care, communication, and observation skills with the ability to work independently while following RN supervision requirements.
Ability to practice within LVN scope of practice and agency policies, as well as state and federal regulations governing home health services.
