Home Health Occupational Therapist

Help patients live more independently in the comfort of their own homes. In this role, you'll deliver skilled occupational therapy services that improve daily function, support aging in place, and enhance overall quality of life. Your expertise will play a critical role in helping patients regain confidence, mobility, and independence.

Your Day-to-Day

• Assess patients' functional abilities and develop individualized treatment plans focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Deliver skilled occupational therapy interventions in the home setting.
• Apply rehabilitation techniques for patients with complex neurologic injuries and conditions.
• Educate patients and caregivers on adaptive strategies, assistive devices, and home modifications that promote safety and independence.
• Collaborate with nurses, physical therapists, physicians, and other interdisciplinary team members to support patient outcomes.
• Document evaluations, treatment plans, progress, and outcomes in accordance with regulatory and company standards.
• Travel throughout the designated service area to provide in-home patient care.

What We're Looking For

• Master's degree in Occupational Therapy (or equivalent).
• Active state occupational therapy licensure or certification in the state of service.
• Specialized knowledge of rehabilitation techniques for complex neurologic injuries.
• Experience in home health, medical-surgical, or rehabilitation center settings.
• Minimum of two years of experience working independently with minimal supervision.
• Valid driver's license and ability to travel throughout assigned business locations as needed.
