Home Health Occupational Therapist (OT) & Certified Occupational Therapy Assistant (COTA) – Per Diem / PRN

OT Pay: $80 per visit or $90,000–$110,000 annually (based on caseload)

COTA Pay: $60 per visit

Location: O‘ahu, HI (Reliable transportation required)

Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload

What You'll Do

• 
  

  Provide skilled occupational therapy services in patients' homes

  
  


• 
  

  Perform initial evaluations, reassessments, and fall risk assessments (OT)

  
  


• 
  

  Develop and implement individualized treatment plans

  
  


• 
  

  Improve independence with activities of daily living (ADLs), mobility, safety, and home management tasks

  
  


• 
  

  Educate patients and caregivers on adaptive techniques and fall prevention strategies

  
  


• 
  

  Collaborate with nurses, therapists, physicians, and caregivers

  
  


• 
  

  Maintain accurate documentation in accordance with Medicare home health standards

  
  

What We're Looking For

• 
  

  Current Hawaii license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA)

  
  


• 
  

  Minimum one year of clinical experience (home health experience preferred)

  
  


• 
  

  Strong communication, organizational, and documentation skills

  
  


• 
  

  Ability to work independently in the field

  
  


• 
  

  Valid driver's license and reliable transportation

  
  


•
