The Role

As a PRN OT with StellarCare, you will provide high-quality home
health services to patients, helping them regain independence in the
activities that matter most. You pick the coverage areas and the days
you want to work—we handle the rest.

What You’ll Do

• Restore Independence: Conduct initial evaluations and
  develop goal-oriented plans focused on ADLs (Activities of Daily
  Living) and IADLs.
  

• Clinical Excellence: Provide one-on-one therapeutic
  interventions, including fine motor coordination, cognitive
  retraining, adaptive equipment training, and activities within
  your scope of practice.
  

• Environmental Mastery: Perform comprehensive home safety
  assessments and recommend modifications or specialized equipment
  to foster aging-in-place.
  

• Documentation: Complete timely OASIS assessments and
  progress notes via our intuitive EMR system.
  

• Collaborate: Work closely with physicians and the
  StellarCare clinical team to ensure seamless patient transitions
  and holistic care.

What You Bring

• Credentials: A degree in Occupational Therapy from an
  accredited program, NBCOT certification, and a valid state license.
  

• Experience: Minimum of 1 year of clinical experience (Home
  Health experience preferred).
  

• Reliability: A valid driver’s license, auto insurance, and
  dependable transportation.
  

• Tech-Savvy: Comfort with electronic documentation and
