Your Day-to-Day

• Perform comprehensive physical assessments and review medical histories to accurately evaluate patient conditions.
• Develop and implement individualized treatment plans aligned with each patient's rehabilitation goals.
• Lead therapeutic exercise programs and mobility training to improve function and independence.
• Educate patients and family members on injury prevention, wellness strategies, and recovery expectations.
• Monitor patient progress and modify treatment plans as needed to optimize outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to support coordinated patient care.
• Maintain accurate, timely, and compliant patient documentation and records.
• Travel to patient homes throughout the service area to deliver high-quality physical therapy services.

What We're Looking For

• Minimum of nine months of experience as a Physical Therapist.
• Home health or community-based care experience preferred.
• Master's degree in Physical Therapy from a CAPTE-accredited physical therapy education program.
• Current Physical Therapist license in the state of agency operation and compliance with accepted professional standards and practices.
• Valid CPR certification required and maintained throughout employment.
• Valid driver's license and ability to travel to patient locations as needed.

Compensation: $101,000 – $112,000 annually
