What Will You Be Doing?

As a Physical Therapist with Gallagher Home Health, you will evaluate, plan, coordinate, and provide patient care based on the Physician’s Plan of Treatment. You’ll have the chance to make a direct impact on patients’ lives while working in the comfort of their homes.

Responsibilities

• Provide comprehensive patient care through assessment and treatment planning in home health settings.


• Utilize knowledge of physiology and anatomy to develop effective physical therapy interventions.


• Collaborate with healthcare teams to create and implement care plans, ensuring continuity of care.


• Educate patients and families on therapeutic exercises and discharge planning.


• Maintain accurate medical records and documentation in accordance with regulations.

What We’re Looking For:

• Active Physical Therapist license in the State of Pennsylvania


• At least 1 year of recent experience (Home Health experience preferred)


• Valid Pennsylvania Driver’s License with no restrictions and a car with required insurance


• Satisfactory Criminal Record Clearance
